How we will report work — honestly, including what remains unknown.
This is the public structure every Regenera Case Note follows. It is a template, not a case study.
No Case Notes are published yet. Case Notes only appear when the client or partner has given permission and there is sufficient evidence to report.
1. Challenge / baseline
The place, the problem, and what was recorded before any action.
2. Intervention
The bounded action taken, who did what, and over what period.
3. Evidence collected
What was recorded, by what method, with sources and limitations.
4. What changed
Observed or measured change — distinguishing activity, output, outcome and contribution.
5. What remains unknown
Gaps, confounding factors and questions the evidence cannot answer yet. Absence of a reported outcome is not treated as failure or success; it is reported as unknown where evidence is insufficient.
6. Learning
What worked, what did not, and what we would change.
7. Next decision
The decision the evidence supports — continue, adjust, scale or stop.
